The thirst for immortality (that very Kundera-esque one) unsettles us and draws human gaze beyond the horizon of longevity. Anti-age therapy and beauty injections for rejuvenation no longer inspire the same admiration, for a new «live longer» pill has now become far more desirable. In pursuit of longevity, people are ready to drink collagen «healing» smoothies and eat protein bars, yet often remain unwilling to change their mindset or their approach to life. There is no magical recipe for achieving quality longevity. The «Blue Zone», such as Okinawa and Sardinia, for instance, do not possess a single «secret» ingredient of long life. So let us try to approach the question of health from a long-term perspective and from a constructive position.

 

THE HEART AS AN EXISTENTIAL INDICATOR OF LONGEVITY

s a doctor, I know for certain that our heart is extraordinary. In a year, it makes 36–37 million beats, pumps 2.5 million liters of blood, and sustains life itself. Therefore, when it comes to longevity, it is quite logical that the state of the heart plays an existential role. You may be surprised, but the age of your heart is not necessarily identical to your chronological age — that is, the number of years you have lived. In fact, the heart may be older or younger than you depending on your lifestyle and health condition. Let us delve into the most intimate meanings of our physical existence. Recall Dorian Gray: Wilde’s hero remains outwardly a handsome young man, while his portrait ages, absorbing the consequences of all his decisions.

Cardiologists have a similar analogy: your calendar age does not always coincide with your cardiovascular age — the portrait of your heart created by the mathematics of your life. Everyone can learn the age of their heart not from social media tests, but with the help of special calculators capable of predicting quality longevity. We ourselves can grasp the number that reflects how much older or younger the heart is than our passport age. This number is not a metaphor but a risk assessment. It was created to help us understand that the aging of the heart is not only about years, but about a complex of factors that we can change. And perhaps for this very reason cardiovascular age is becoming a powerful tool for those who are ready to take responsibility for their health.

 

HOW THE ASSESSMENT OF HEART AGE WORKS

 

Cardiovascular age is a way of translating complex statistical data about the risk of cardiovascular events into a language that is easy to understand. Essentially, it is a model that compares the condition of your heart with that of a hypothetical «ideal peer» — a person of the same age but without risk factors: normal blood pressure, acceptable cholesterol levels, no smoking, and no diabetes. If your indicators are worse, the model adds years to your heart’s age; if they are better, it makes the heart younger. Therefore, cardiovascular age is not just a number, but a clear portrait of the state of the cardiovascular system, one that changes depending on our lifestyle. For example, if you are 50 years old but your heart age is 56, this means that your risk of disease corresponds to the level of a healthy 56-year-old person.

The fewer the risk factors, the closer the heart age is to the real age, and sometimes it may even be younger. Studies show that presenting risk in the format of «heart age» can better motivate people to make changes — especially middle-aged patients. When risk becomes personalized and understandable, it is easier to recognize the need for prevention, although emotional reactions to such information may vary. Imagine that your «heart portrait» is alive and responds to routine everyday actions: controlling blood pressure, quitting smoking, regular walks or physical activity — even small steps can literally «rejuvenate» your heart by several years.

 

 

FACTORS THAT SHAPE CARDIOVASCULAR AGE

 

Blood Pressure

 

Chronic elevation of blood pressure is associated with left ventricular remodeling, an increase in cardiac mass, and a higher risk of cardiovascular events. Even moderate changes in blood pressure can significantly influence «cardiovascular age» and are prognostically important.

 

Lipid Profile

 

Elevated LDL cholesterol and an imbalance of lipoproteins create conditions for atherosclerosis, which directly increases cardiovascular risk and accelerates the aging of the vascular wall.

 

Smoking

 

Tobacco smoking leads to endothelial dysfunction, activation of inflammatory mechanisms, and oxidative stress, indirectly worsening vascular health.

 

Metabolic Disorders

 

Diabetes and obesity are associated with insulin resistance, chronic inflammation, and vascular damage, increasing cardiovascular age. Control of body weight and glycemia is critically important for reducing the risk of complications.

 

Physical Activity and Sleep

 

Regular aerobic exercise and sufficient sleep improve endothelial function, reduce blood pressure, and support autonomic balance. These factors are directly correlated with the preservation of vascular elasticity and cardiac function.

 

Genetics

 

Polygenic assessment (Polygenic Risk Scores) makes it possible to evaluate the predisposition to cardiovascular diseases even before clinical manifestations appear. Genetic risk is particularly important in younger patients, when traditional risk factors have not yet become evident. At the same time, a healthy lifestyle can modify the expression of genetic risks, slowing the progression of pathological changes in the cardiovascular system.

Today, there are many tools for monitoring and responding quickly to heart-related challenges: «aging clocks» with integrated AI, measurement of arterial stiffness, assessment of endothelial function, analysis of heart rate variability, evaluation of retinal condition, and 12-lead ECG. Cardiovascular age is a mirror of possibilities. It combines medicine, mathematics, and psychology, helping to predict the risk of cardiac events and motivating change. The portrait of your heart reveals aging earlier than you may feel it, yet it can be altered. Every lifestyle decision is a chance to turn back time and write your own story of longevity.
